The PMC Repair Process
1. Download a Repair Request Form.
2. Fill out all the information on the form except for the RMA number. This will be given to you by the
PMC Repair Dept.
3. Call Parks Medical at (509) 448-9112 and request an RMA (return merchandise authorization) number.
4. Be prepared to give the scope’s serial number and describe the problem.
5. Once the information is added to our system, PMC will give you an RMA number.
Please add this information to your repair request form.
6. Place the repair request form and the scope in a shipping box. Ship the scope to PMC for diagnosis
and repair upon your approval.
